All requests for leave should be submitted by the employee as far in advance as possible. Typically, for foreseeable events, employees should give 30 days advance notice of the need to take leave. If the employee does not know precisely when the leave will be required, the employee must notify the department of the need for leave as soon as possible.

Employees requesting to use annual leave of up to thirty (30) days for vacation purposes, must apply for a Personal Leave of Absence.

Eligible employees needing to take time off for medical reasons may apply to take up to 12 workweeks of unpaid, job-protected leave under the Family and Medical Leave Act (FMLA).

WHAT IS FMLA?

The FMLA is a federal law that provides eligible employees with unpaid, job-protected leave for specific medical and family reasons.

Eligible employees may take up to 12 workweeks of unpaid leave per fiscal year (July 1 – June 30).

To be eligible for FMLA, you must meet the following requirements:

  1. Have at least 12 months of employment with UCF; and
  2. Have worked at least 1,250 hours for UCF during the 12 months prior to your request; and
  3. Did not exhaust your current FMLA entitlement.

EMPLOYEES MAY APPLY FOR FMLA LEAVE FOR ONE OF THE FOUR REASONS:

  1. Full Continuous Leave: The need to take time off consecutively or all at once for specified medical reasons.
  2. Intermittent Leave: The need to take time off occasionally for specified medical reasons.
  3. Parental Leave: The need to take time off for the birth of a child or placement with the employee for adoption or foster care.
  4. A Reduced Work Schedule: The need to reduce the amount of hours worked in the usual daily or weekly work schedule for specific medical reasons.

HOW DO I APPLY FOR FMLA?

  1. Complete the Medical Leave Request Form
  2. Email the form to fshr@ucf.edu
  3. FSHR will route the form for department signatures
  4. FSHR will contact the employee to discuss the process and provide additional required paperwork
